Explains how we protect and use your health information, when information may be shared, and your rights regarding access, privacy, restrictions, and complaints. This notice also explains the additional protections that apply to certain mental health records.
​Reviews our billing and payment policies, including insurance responsibility, card-on-file requirements, copays and deductibles, cancellation and no-show fees, outstanding balances, and payment arrangements. We encourage clients to review this information before beginning services.
Explains what to expect from therapy at Roswell MindCare, including consent to treatment, confidentiality, scheduling and telehealth expectations, communication, practice policies, and circumstances in which services may be ended.
Intake Assessments
Standardized assessments come in all shapes and sizes and can be used to help with diagnosis, symptom tracking, and more. At RMC, there are a few assessments we send before intake to get a snapshot of the individual's symptoms and needs. Clinicians may substitute assessments, assign additional assessments, or forego assessments as your treatment progresses to find the best fit for you. Assessments are best completed through the portal and includes progress tracking, but paper alternatives are available for those who require them.

Together, these intake assessments take 5-15 minutes to complete:
PCL-5 - Trauma based disorders have become the most common that we see in our practice. This PTSD Checklist for DSM-5 catalogs the symptom severity across the many different potential symptoms of posttraumatic stress disorder and related conditions.
DASS-21 - If you've participated in therapy, you've likely done the GAD-7 or PHQ-9. At RMC, we use this 21 item Depression, Anxiety, and Stress Scale to get an overview of these symptoms and which ones are most relevant to the individual.
AUDIT-C Plus 3 - Substance use disorders are an unfortunate fact of our modern world and affects persons across the state, country, and globe. There are many versions and options for assessing, but at RMC we typically utilize this variant of the Alcohol Use Disorder Identification Test to screen for current use and risk factors.
C-SSRS - The Columbia-Suicide Severity Rating Scale is widely utilized as the standard for assessing the most common progression of suicidal ideation (SI).
ACE - The Adverse Childhood Experience Questionnaire has been used since the 90s to identify 10 of the most common destabilizing events experienced by children.
